About the role

We are seeking a compassionate Licensed Vocational Nurse/Licensed Practical Nurse (LVN/LPN) who enjoys providing personalized care in the comfort of the patient's home. You will join a team dedicated to delivering exceptional home health services while supporting one another through collaboration, respect and excellence.

Responsibilities

• Provide skilled nursing services under the direction of an RN
• Administer medications and perform wound care
• Educate patients and monitor changes in condition
• Document patient progress
• Help patients maintain independence while promoting comfort, safety, and healing
• Work alongside nurses, therapists, physicians, and caregivers to achieve positive patient outcomes
• Build meaningful relationships with patients and families while promoting independence, improving outcomes, and ensuring compassionate, patient centered care

Qualifications

• Current California Licensed Vocational Nurse (LVN) license
• Current BLS certification
• Valid driver's license, reliable transportation, and auto insurance
• Excellent assessment, communication, and critical thinking skills
• Ability to work independently while collaborating with an interdisciplinary team

Pay

$35.00–$45.00 per hour, based on experience.
