About the role

We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide personalized care in the comfort of the patient’s home. Join a team dedicated to delivering exceptional home health services while supporting one another through collaboration, respect and excellence.

Responsibilities

• Provide skilled nursing services under the direction of an RN, including medication administration, wound care, and patient education
• Monitor changes in patient condition and document patient progress
• Maintain patient comfort, safety, and healing while promoting independence
• Collaborate with nurses, therapists, physicians, and caregivers to achieve positive patient outcomes
• Build meaningful relationships with patients and families while promoting independence and improving outcomes

Requirements

• Current California Licensed Vocational Nurse (LVN) license
• Current BLS certification
• Valid driver’s license, reliable transportation, and auto insurance
• Excellent assessment, communication, and critical thinking skills
• Ability to work independently while collaborating with an interdisciplinary team

Qualifications

• Previous experience working in home health, hospice, or a similar caregiving environment
• Familiarity with KanTime Electronic Medical Record (EMR) system
• Bilingual skills are a plus
• Wound care/Wound Vac experience

Pay

Salary Range: $35.00–$45.00 per hour, based on experience
