Essentials Duties & Responsibilities
- Assist a Nurse Case Manager in providing direct patient care.
- Initiate communication with team members, HHAs, and other healthcare providers to facilitate optimal care.
- Instruct primary caregivers, patients, volunteers, and employed caregivers in care provision based on the Plan of Care as requested by the Nurse Case Manager or Team Leader.
- Perform nursing procedures within the scope of a Licensed Vocational Nurse.
- Maintain current documentation in a computerized clinical documentation system according to organization policies, procedures, and applicable laws and regulations.
Required Qualifications
- Graduate of an approved and certified LVN program.
- Current and unencumbered license to practice as an LVN in the state.
- Ability to make quick judgments and work independently.
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines.
- At least one year of LVN clinical experience; home health experience preferred.
- Ability to effectively utilize EMR and other technology to maintain HIPAA privacy, continuity of care, and team communication.
Skills & Attributes

- High level of organization and time management to navigate a daily route.
Nice to Have
- Experience in acute care and working in a home environment.
- Wound care experience in wound care/vac, post-surgical care, catheterization.
- Experience with KanTime EMR.
