About the role

We are seeking a compassionate Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to provide personalized care in patients’ homes. Join a team dedicated to delivering exceptional home health services while supporting one another through collaboration, respect, and excellence.

Responsibilities

• Provide skilled nursing services under the direction of an RN, including medication administration, wound care, and patient education.
• Monitor changes in patient condition and document progress.
• Help patients maintain independence while promoting comfort, safety, and healing.
• Collaborate with nurses, therapists, physicians, and caregivers to achieve positive patient outcomes.
• Build meaningful relationships with patients and families while promoting independence and compassionate, patient-centered care.

Requirements

• Current California Licensed Vocational Nurse (LVN) license.
• Current BLS certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Excellent assessment, communication, and critical thinking skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Qualifications

• Previous experience working in home health, hospice, or a similar caregiving environment.
• Familiarity with KanTime Electronic Medical Record (EMR) system.
• Bilingual skills are a plus.
• Wound care/Wound Vac experience.

Pay

Salary Range: $35.00–$45.00 per hour (based on experience).
