Non-Medical Care Manager

About the role

A Non-Medical Care Manager contributes to AHF by providing coordination, guidance, and assistance in improving access to and retention of needed medical support to mitigate and eliminate barriers to HIV care services. With a caring and knowledgeable focus, you will work with our providers and healthcare center team so that every Ryan White client receives the support they require and access to available resources.

Responsibilities

• Completing Initial Assessments and developing Individualized Care Plans related to psychosocial needs and goals.
• Understanding and addressing basic needs like housing, food, and clothing, and facilitating referrals, support, and assistance for clients who lack these services.
• Conducting ongoing retention calls to ensure no gaps in retention efforts and to uphold cooperative relationships across divisions.
• Maintaining a passion for our top initiatives and ensuring the AHF core values and mission are at the forefront of all actions.
• Collaborating with the provider and patient to implement the optimum plan of care for non-medical needs and putting it into action.
• Providing brief interventions focused on long-term achievable goals, working closely with the patient to achieve viral suppression, better health outcomes, and quality of life.

Requirements

• A High School Diploma required.
• A Bachelor degree in health or human services-related field preferred.
• A minimum of 1 year Case Management experience is preferred.
• Knowledge of Ryan White Services is preferable.
