About the role

Under the general supervision of the Charge Nurse and in accordance with facility policies and procedures, this position provides direct care to residents, assists with activities of daily living, and reports needs and concerns to the supervisor on duty.

Responsibilities

  • Performs or assists with resident direct care needs and comforts of daily living, including lifting, turning, positioning, transporting, bathing, dressing, oral hygiene, grooming, and responding to call lights.
  • Provides restorative care such as range of motion exercises, ambulation, and bowel and bladder training.
  • Maintains cleanliness of resident rooms, including daily cleaning, deep cleaning, and discharge cleaning.
  • Engages residents in activities of interest and provides restorative nursing programs to maintain ADLs.
  • Communicates professionally and effectively with residents, peers, families, managers, and visitors.
  • Reports changes in residents’ physical or mental conditions in a timely manner.
  • Assists nurses with tasks such as measuring and recording vitals, gathering documentation, making beds, cleaning living areas, assisting with new admissions, and training new staff.
  • Adheres to Resident Rights documentation, maintains confidentiality, and reports resident complaints to the Charge Nurse or supervisor.
  • Documents resident information accurately and confidentially in the electronic medical record system.
  • Follows safety, compliance, infection control, and universal precautions protocols.
  • Performs food service functions, including preparing residents for meals, serving food, assisting with eating, after-meal care, passing snacks and water/ice, and recording food/fluid intake.

  • Ensures all care is provided with respect and dignity for residents, families, and staff.
  • Reports resident complaints, allegations of abuse, misappropriation of funds or property, and any compliance concerns to appropriate supervisors.
  • Completes annual education requirements, including Compliance, Privacy, Security, and HIPAA training.

Requirements

  • High school diploma or GED preferred, or currently enrolled in a program to achieve preferred education.
  • Two to four years of relevant experience preferred.
  • Good communication, comprehension, and interpersonal skills; ability to speak, read, write, and understand English.
  • Working knowledge of Windows, Microsoft Office Suite, and the World Wide Web.

Physical demands

  • Lifting/Carrying: Frequent 10–25 lbs, Frequent 26–50 lbs, Sometimes 51–75 lbs, Frequent 76+ lbs.
  • Frequent standing, walking, pushing, pulling, stooping, and lifting.
  • Sometimes sitting; rarely climbing or driving.
  • Exposure to noise, odors, illness, infection, and physical contact with disruptive residents.
