About the role

The Navigator I role at Community Bridges, Inc. (CBI) involves enhancing the likelihood of success for clients, identifying and reducing barriers, and connecting them with necessary services and resources. This role requires conducting outreach, engagement, case management, and providing ongoing wrap-around supportive services. The Navigator I works within a multi-disciplinary team comprising internal and external stakeholders.

Responsibilities

• Enhance the likelihood of success for clients by identifying and reducing barriers.
• Conduct outreach and engage with individuals needing additional support.
• Manage cases and provide ongoing wrap-around supportive services.
• Map out the most appropriate next steps for individuals needing assistance in accessing various services.
• Document and track required patient interactions according to Arizona Administrative Code, ADHS/DBHS Provider Manual, CARF, Community Bridges Policies and Procedures, and specific program guidelines.

Requirements

• A high school diploma or GED is required.
• An associates degree (or higher) in a field related to behavioral health is preferred but not required.
• Minimum of six months of recovery from substance use and/or mental health disorders is preferred.
• One to three years of full-time health care-related work experience, preferably involving behavioral and medical crisis situations, is preferred but not required.
• One to three years of substance abuse/mental health recovery-oriented service experience is preferred but not required.
• A valid and good standing driver's license is required.
• A clear 39-month motor vehicle record is required.
