NAVIGATE Program Coordinator

M Physicians is affiliated with the University of Minnesota Department of Psychiatry and Behavioral Sciences, a leader in first episode psychosis care and research. The NAVIGATE program, an evidence-based coordinated specialty care initiative, began in 2017 with grant support from the Minnesota Department of Human Services. This interdisciplinary, team-based program focuses on resiliency and recovery for individuals ages 15 to 40 with first episode psychosis in schizophrenia spectrum disorders. NAVIGATE provides a range of interventions, including medication management, individual resiliency training (IRT), supported employment and education (SEE), peer support, case management, care coordination, and family support and education. Team members deliver both clinical and non-clinical services in clinics, communities, and patients' homes, collaborating closely in weekly team meetings and receiving one-to-one supervision from the program’s director.

Responsibilities

• Attend all NAVIGATE team meetings, record meeting minutes, and track team member action items.
• Develop and maintain up-to-date program policies and workflows.
• Assist with coordinating and onboarding new team members and learners.
• Support team members with implementing and maintaining special projects, events, or initiatives (e.g., groups, workshops, program marketing, and community education).
• Answer, coordinate, and facilitate responses to program referrals and requests for more information.
• Coordinate receipt of needed consents for services and releases of information, and obtain internal and external records whenever possible.
• Secure data, facts, and information to ensure appropriate scheduling and insurance reimbursements.
• Support team members with patient and family care coordination, including setting program expectations.
• Monitor NAVIGATE visit frequency and assist with scheduling appointments and transportation to/from appointments.
• Monitor progression of NAVIGATE services from enrollment to graduation/discharge, keeping the team apprised of patient enrollment length and facilitating the graduation/discharge process.
• Use appropriate techniques for handling urgent patient issues, including urgent evaluation appointments per clinic protocol.
• Answer the mental health NAVIGATE phone line as possible.
• Maintain timely and accurate notes in Epic and other systems.
• Communicate effectively with other members of the healthcare team, including summarizing complex patient situations.

Requirements

• Bachelor's Degree in Psychology, Social Work, or related Social Sciences field.
• 2+ years of experience working with individuals impacted by mental illness, including but not limited to psychosis.
• This role may require availability beyond traditional office hours.
• Comfort working directly in the community with clients, including home and community visits.

Location

St. Louis Park Clinic - 5775 Wayzata Blvd Suite 200, St. Louis Park, MN 55416. This is a hybrid position with work locations including the clinic (St. Louis Park Psychiatry Clinic), community settings (e.g., coffee shops, libraries, patient homes), and home. The percentage of time spent in clinic or community is based on patient need and preference.

Pay

$53,570.00 - $77,672.00 USD annually.
