About the role

Provides biopsychosocial assessment, crisis intervention, short-term counseling, advocacy, and linkage to resources and planning for transitions of care for patients and their families/significant others of all ages in any patient care setting. Provides psycho-education and may facilitate support groups. Provides consultation on psychosocial aspects of care as a member of the interdisciplinary team; serves as a liaison to community programs and collaborates with the multidisciplinary treatment on discharge planning. May provide education to staff and other hospital departments and participates on committees as requested.

Responsibilities

• Perform biopsychosocial assessments and clinical interventions, including crisis intervention, patient/family behavioral issues, grief counseling, supportive counseling, adjustment to illness, life review/end-of-life support, behavioral change therapy, motivational interviewing, short-term family counseling, and group facilitation.
• Collaborate effectively as part of an interdisciplinary team.
• Advocate for patients and families.
• Coordinate transitions of care and discharge planning, including knowledge of post-acute levels of care and resource needs.
• Assess mental health functioning, high-risk behaviors, depression, anxiety, and other psychiatric conditions impacting hospitalization or transitions of care.
• Assess suicidal behavior, lethality, and develop appropriate plans of care.
• Assess substance use, level of addiction, and motivation for change; develop and implement appropriate plans of care.
• Provide culturally and spiritually sensitive psychosocial care appropriate to age-specific needs (newborns, children, adolescents, adults, and elders).
• Conduct goals of care discussions and address psychosocial needs of patients and families at end-of-life.
• Navigate bioethical and legal issues impacting patient care.
• Communicate effectively both verbally and in writing.
• Demonstrate strong interpersonal and organizational skills in a fast-paced environment with rapidly changing priorities.
• Utilize computer software for data entry, email, scheduling, and task management.
• Liaise with local county, state, and federal resources for at-risk populations.
• Comply with reporting requirements for child, elder, and dependent adult abuse, crimes against persons, and other regulations (e.g., Tarasoff, patient confidentiality).

Requirements

• Master’s degree in Social Work (MSW) or Masters of Social Services (MSS) from a school accredited by the Council on Social Work Education.
• For LCSW III: Licensed Clinical Social Worker (LCSW) certification.
• 2 years of recent relevant experience.

Pay

MSW II: $50.82 - $71.15 per hour

LCSW III: $53.36 - $74.70 per hour

The compensation range may vary based on geographic location, candidate experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.

Schedule

• Per Diem/Casual (variable hours, as needed).
• Rotating weekends (every 3rd or 4th weekend).
• 8-hour shifts on variable days of the week.
