About the role
CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals.
Responsibilities
- Ensure timely and efficient delivery of mobile services to target populations; oversee the daily schedule/calendar and operations of the mobile program;
- Coordinate and obtain appropriate consents for treatment and documentation required from parent/guardian(s);
- Coordinate/oversee the scheduling of appointments for treatment; ensure adequate number of patients are scheduled;
- Facilitate the transport of patients/potential patients to and from the mobile clinic;
- Support with weekly and monthly staffing coverage to ensure care is not impacted;
- Serve as a liaison between CommuniCare+OLE and the sites to be serviced;
- Identify community resources in the service area, and establish partnerships with identified organizations;
- Ensure parking arrangements; ensure the safety of the mobile clinic and compliance with all risk management policies and guidelines;
- Provide a secure and clean environment on the mobile unit; ensure proper handling and disposal of hazardous material;
- Manage the details around Specialty RV management;
- Manage the daily log of vehicle mileage and compressor hours; complete driver vehicle evaluation form weekly;
- Maintain a professional relationship and positive attitude with co-workers, the public, patients, and the entire organization;
- Secure permits and necessary approvals for the mobile unit operations at different locations;
- Coordinate transportation, logistics, and maintenance of the mobile unit;
- Maintain accurate patient records and clinical data for the program;
- Generate reports on patient demographics, service utilization and KPI;
- Monitor the quality of care provided on the mobile unit, identifying areas for improvement;
- Identify and implement quality assurance measures to enhance patient experience and clinical outcomes;
- Support goals and objectives of all involved.
Requirements
- Education: Bachelor's degree in public health, healthcare administration, or related field preferred. High school diploma required.
- Experience/Lived Experience: Minimum of 2 years of experience managing a health clinic or program strongly preferred.
- Special Skills/Training: Able to communicate fluently in English and Spanish (Strongly Preferred).
- Other: Valid CA Driver’s License required.
Qualifications
- BLC Certification required.
Skills
- Able to communicate effectively with various stakeholders including patients, community partners, the community, and staff and management.
- Knowledge of community health/public health philosophy and operating principles.
- Knowledge of the different community stakeholders (community partners, vulnerable and hard-to-reach communities, etc.).
- Strong computer and data entry skills preferred, as well as knowledge of Microsoft suite (Word, Excel, Power Point, Outlook, etc.), and healthcare data management systems.

Pay Range: $31.37 to $38.34/hr
Schedule
Monday to Friday, 40 hours a week in Napa, CA
