The Role
The role of the Mobile Crisis Case Manager is to provide critical support to individuals facing mental health and related crisis in our community.
What you will do
- Respond to mobile crisis dispatches and provide on-site de-escalation and suicide risk screening, safety planning, and resource linkage
- Offer empathetic listening and emotional support to individuals in distress
- Plans for and provides effective, understandable, and respectful service to both staff and consumers that are compatible with an individual's beliefs
- Generates accurate and timely documentation of service provision, provides accurate and timely summaries of client progress and/or service activities, which results in high quality records meeting all applicable standards and procedures
- Follows up with clients, make referrals, and collaborate with community partners and treatment providers
- Maintains compliance with contractual obligations for the program that allows the program and organization to legally bill for services
- Provides linkage, monitoring/follow-up, referral, advocacy and crisis intervention and sufficient follow-up to ensure that clients are connected to needed services and supports both
- Performs other related duties as assigned
Qualifications
- Bachelor's degree in human services arena (Psychology, or Social Work) required
- At least 1 year of professional-level experience in human relations or related field is preferred
Employment Requirements
- Completion of a pre-employment drug screen
- Completion of a background check
- Satisfactory Driving Record
