About the role

Supports seamless, patient-centered care behind the scenes. Works onsite at a Nascentia Health office, Monday-Friday, 8:00 AM - 4:30 PM.

Responsibilities

  • Support efficient, accurate care coordination through strong organizational and communication skills.
  • Review, process, and enter authorization data accurately into computerized systems.
  • Monitor and manage expiring authorization reports to ensure continuity of services.
  • Absorb and coordinate Member services as directed by Care Managers.
  • Collaborate regularly with clinical team members and participate in team meetings.
  • Act as a resource to Care Managers, supporting scheduling and tracking of Member contacts, visits, and UAS assessments.
  • Answer and triage phone calls, respond to questions, and escalate issues as appropriate.
  • Communicate with Members on a regular basis to support care coordination needs.
  • Perform clerical and administrative tasks including filing, copying, faxing, typing, and message-taking.
  • Participate in performance improvement and continuous quality improvement (CQI) initiatives.
  • Perform other duties as assigned.

Requirements

  • High school diploma required; Associates degree in a related field preferred.
  • Two or more years of experience in healthcare or completion of a Medical Assistant program.
  • Strong computer literacy with the ability and interest to learn new systems and technologies.
  • Proficiency in Microsoft Office, internet-based tools, and healthcare software applications.
  • Strong analytical and problem-solving skills.
  • Ability to work independently while collaborating effectively with a team.
  • Excellent verbal and written communication skills.
  • Demonstrated discretion in handling confidential and sensitive information.
  • Ability to adapt to changing priorities, deadlines, and workflows.

Qualifications
