About the role

This position covers both Monarch Healthcare Management's Waterview Woods location in Eveleth, MN and Waterview Shores location in Two Harbors, MN.

Responsibilities

  • Oversight of the completion, editing, locking and transmission process of all MDS assessments.
  • Focus on accuracy, case mix maximization, and regulatory compliance.
  • Assure timely completion of the MDS.
  • Coordinate care plan completion with the Interdisciplinary Team (IDT).
  • Render clinical support to nursing staff to enhance accurate assessments, care plans, and excellent quality of care.

Qualifications

  • Must be a Registered nurse (RN) or Licensed Practical Nurse (LPN) in the state of Minnesota.
  • Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.
  • Must possess the ability to communicate verbally and in writing when directing the Resident Assessment Instrument (RAI) process and when performing and assessing care for residents.
  • Experience in healthcare, skilled nursing facility, or assisted living community.
  • Experience with MDS and Resident Assessment Instrument (RAI).
