Minimum Data Set (MDS) Coordinator
Position Summary
We are pleased to offer the current job opportunity for a Minimum Date Set (MDS) Coordinator at Kenneth Moy DuPage Care Center. Responsibilities
- Coincides with other departments to collect and input the information required for the federally mandated assessments (MDS)
- Maintains input to ensure accuracy, timeliness and compliance with reimbursement authorities and standards
- Monitors clinical charts and condition of patients to plan and coordinate resident's plan of care with other members of the interdisciplinary team
- Communicates with physicians, nursing staff and family members regarding resident conditions
- Completes the computer database required for the MDS
- Oversees documentation to comply with Medicare or Illinois Department of Public Aid reimbursement rules
- Led care plan conferences and other interdisciplinary meetings
- Interprets resident charts to accurately complete sections of the MDS
- Reviews resident charts for contradictions in information and researches any contradictions to determine what is valid
- Stays current with job related professional developments, regulations and standards
- Recommends hires and promotions, directs and evaluates employment decisions for all assigned positions
- Performs the duties of a Registered Nurse as assigned
Requirements
- Completion of coursework for a diploma in Nursing from an accredited college or university
- One (1) year experience working with MDS
- Possession of a current license in Illinois as a Professional Registered Nurse (RN) and a Resident Assessment Coordinator Certification

Physical Requirements: Heavy Work: Exerting up to 50 pounds of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Use of arm and/or leg controls requires exertion of forces greater than for medium work.
Work Environment: Clinical skilled nursing facility and customer service setting with a moderate to quiet noise level.
