Duties

  • Coordinates the facility’s Resident Assessment Instrument (RAI) process in accordance with state and federal regulations.
  • Accurately completes all MDS assessments and any supporting assessments or clinical documentation.
  • Implements, evaluates, and monitors each resident’s comprehensive plan of care.
  • Schedules and leads interdisciplinary care plan team meetings.
  • Audits medical records for the presence of supporting documentation for all items coded on the MDS.
  • Provides audit-related feedback and education to department heads, physicians, and other staff as needed.
  • Collaborates with health plans, physicians and the interdisciplinary team to ensure insurance approvals and/or that residents meet eligibility and coverage criteria.
  • Interprets rules, regulations and coverage guidelines.
  • Serves as primary resource for problem-solving for the Skilled Nursing Facility (SNF) prospective payment system and quality reporting program.
  • Provides a schedule of ARDs and assessment types weekly and as needed to the Interdisciplinary Team (IDT) in order to facilitate the timely completion of MDS sections and CAAs (Care Area Assessments) by each discipline.
  • Ensures the Business Office designee receives the initial and final validation report that acknowledges the acceptance of the MDS into the QIES ASAP.
  • Facilitates the completion and updating of resident care plans with the IDT to reflect each resident’s current needs.
  • Participates in all resource utilization and/or triple check meetings prior to billing of claims associated with payment assessment.
  • Performs other duties as requested or required.

Knowledge, Skills, and Abilities

  • Nursing and medical practices and procedures as well as laws, regulations, and guidelines that govern long-term care.
  • Current state and federal regulations, practice standards, and procedures, as well as guidelines pertaining to the MDS process.
  • Center for Medicare and Medicaid Services (CMS) regulations.
  • Long Term Care (LTC) and Skilled Nursing Facility (SNF) regulations and guidelines.
  • Patient-centered nursing best practices.
  • Electronic health record (EHR) systems.
  • Microsoft Office Suite, including Excel, and basic computing systems.

Knowledge Required

  • Knowledge of nursing and medical practices and procedures as well as laws, regulations, and guidelines that govern long-term care.
  • Knowledge of current state and federal regulations, practice standards, and procedures, as well as guidelines pertaining to the MDS process.

Skills Required

  • Accurately and effectively coordinating and administering technical criteria of MDS responsibilities.
  • Electronic health record (EHR) systems.
  • Microsoft Office Suite, including Excel, and basic computing systems.

Ability Required

  • Communicate effectively, both orally and in writing.
  • Promote safe work practices, safety rules, and accident prevention procedures to prevent employee injury and illness.
  • Meet general health requirements according to facility policy, including medical and physical exams and checking immunity status to various infectious diseases.
  • Foster effective working relationships and build consensus.

  • Collaborate with individuals at all levels of the organization, as well as with residents, family members, visitors, government personnel, and the public.

  • Maintain confidentiality of sensitive information.
  • Aid in evacuation of residents during emergency situations.

Residency: Not required to live in Allegheny County
