About the role

Our brand-new, state-of-the-art facility in Pinehurst, North Carolina (NC), was thoughtfully designed to redefine care through exceptional hospitality, innovative services, and a welcoming environment for both residents and team members. We are currently seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) Minimum Data Set (MDS) Coordinator to join our dedicated team of healthcare professionals.

Responsibilities

  • Captures assessments and care plans for residents using the Resident Assessment Instrument (RAI) process in accordance with the laws, regulations, and company standards.
  • Reviews and coordinates Patient Driven Payment Model (PDPM) and Case Mix data for the facility to ensure appropriate reimbursement.
  • Captures and maintains schedules for all MDSs.
  • Ensures accuracy of MDS data and meets with staff as needed to address any issues.
  • Completes and inputs MDS data into the computer system.
  • Corrects any rejected records and prepares them for re-submission.
  • Formulates and implements Resident Care Plans with the interdisciplinary care plan team.
  • Schedules and conducts Care Plan meetings on a regular and timely basis.
  • Communicates the Care Plan to relevant staff members and reports non-compliance to the supervisor.
  • Collaborates with in-house and ancillary departments to ensure compliance with the Care Plan.
  • Performs other related duties as directed by the Director of Nursing Services and Administrator.

Requirements

  • Current license as an RN or LPN issued by the State Board of Nursing.
  • Experience with the RAI process.
  • Strong knowledge of PDPM and State Case Mix Time Weighted Reimbursement payment system.
  • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to long-term care.
  • Effective verbal and written English communication skills, and customer service orientation.
  • Strong attention to detail and accuracy, organizational skills, and the ability to prioritize and coordinate multiple projects.
  • Function independently, possess flexibility, personal integrity, and the ability to collaborate effectively with employees and vendors.
  • Demonstrate basic to intermediate skills in Microsoft Word, Excel, PowerPoint, and internet navigation (preferred).
  • Maintain confidentiality and exhibit high levels of professionalism.
