Metro Outreach Case Manager

About the role

The Metro Outreach Case Manager position is part of the HOPICS division and is non-exempt with full-time status (40 hours per week). The role is supervised by the Program Supervisor and Program Manager, and involves professional case management services for individuals experiencing homelessness.

Responsibilities

  • Conduct immediate needs assessments with adult consumers to obtain information for comprehensive treatment plans
  • Collaborate with internal and external entities through participation in Multi-Disciplinary Teams (MDTs)
  • Work comfortably with homeless clients in the field
  • Create client treatment plans aligned with clinical assessments from therapists, nurses, and substance use counselors
  • Aid clients in resolving or coping with issues contributing to mental, emotional, medical, and/or substance-related problems
  • Document in accordance with E6 program guidelines, including entering notes and treatment plans into HMIS
  • Provide services primarily to clients living in or on LA County property, such as Metro rail stations, bus stops, and other Metro facilities
  • Offer case management services, including addressing immediate needs, linking to benefits, housing supports, mental health, physical health, and substance use programs, food support, hygiene support, and employment/school resources
  • Work with homeless clients, those with co-occurring substance abuse issues and mental/physical/developmental disabilities, and maintain appropriate boundaries
  • Document interventions and treatments in the field
  • Coordinate transportation and transport E6 staff and participants using the Agency van
  • Keep records of homeless status for intake workers and clients, and maintain copies in client files
  • Provide HIV/AIDS information, education, and linkages to testing, housing, and services
  • Be knowledgeable about HIV/AIDS services and testing locations
  • Enter all outreach activity into HMIS system regularly and ensure accurate entries
  • Support the preparation of LAHSA required reports, including QPR and APR
  • Build rapport with community organizations serving homeless individuals, families, low-income households, and other HOPICS target populations
  • Inform and educate homeless individuals and families about HOPICS programs and services
  • Work evenings, overtime, weekends, or holidays as required
  • Maintain appropriate boundaries and adhere to SSG’s Code of Ethics and HOPICS’ Core Values
  • Represent the Agency and Division in a professional manner at meetings and community events
  • Maintain regular attendance and ensure client records/documentation are submitted or arranged for timely submission upon resignation
  • Perform other duties as assigned by supervisor and Division Director
  • Support administrative and data-related IT department functions

Qualifications

  • Some college credits or at least two years of experience working with homeless individuals
  • Ability to work with clients from diverse backgrounds
  • Comfortable working with homeless clients in the field
  • Effective communication skills, both written and oral
  • Professional appearance and attention to detail
  • Working knowledge of Microsoft Word, Excel, and Internet
  • Experience working with homeless persons with co-occurring issues (mental/health/disabilities/substances)
  • Ability to work with co-occurring populations that present with any stand-alone or combination of mental illness, health conditions, and substance-related concerns
  • Knowledge of evidence-based practices and ability to use them in the field

This position does not have any supervisory responsibilities.
