About the Role

Under the supervision of the Program Supervisor and Program Manager and as part of a multidisciplinary team, the Outreach Case Manager will perform professional case management services in connection with outreach to, and linkage for, homeless consumers. This role assists consumers and their significant figures in understanding and developing solutions to problems that lead to and result from homelessness. The incumbent will receive technical guidance, consultation, and supervision to enhance their case management skills and will comply with all ethical and legal mandates for documentation and reporting. Evidence-based case management methods will be utilized to assess clients' immediate needs, housing situations, and to create treatment plans addressing issues contributing to homelessness.

The incumbent must coordinate daily fieldwork to service clients, complete documentation, attend meetings, and participate in trainings while working as part of a homeless outreach effort in Service Planning Area 6.

Responsibilities

  • Conduct immediate needs assessments with adult consumers to complete comprehensive treatment plans.
  • Collaborate within a Multi-Disciplinary Team (MDT) with internal and external entities.
  • Provide assistance to homeless clients in the field and establish client treatment plans correlating with clinical assessments from therapists, nurses, and substance use counselors.
  • Assist consumers in resolving or coping with issues contributing to mental, emotional, medical, and/or substance-related problems.
  • Document in accordance with E6 program guidelines, including entering notes, treatment plans, and outcomes into HMIS.
  • Provide case management services to clients living in or on LA County properties (e.g., libraries, parks, pools) including linkages to benefits, housing supports, mental/physical health programs, substance use programs, food/hygiene support, employment, and school resources.
  • Work with homeless consumers, adults with co-occurring substance abuse issues, and those with mental/physical/developmental disabilities.
  • Document effectively in accordance with established standards of care on a daily basis.
  • Participate in required meetings, trainings, collaborations, and multi-disciplinary team meetings; present or evaluate client progress as needed.
  • Coordinate program activities to meet contractual agreements, laws, regulations, and funding requirements with the E6 Program Manager.
  • Attend weekly supervision (individual and group) and other tasks assigned by the E6 Program Manager.
  • Conduct all interventions and treatments in the field.
  • Participate in mandatory program, division meetings, and trainings (including offsite).
  • Coordinate transportation and transport E6 staff and participants using the Agency van as needed.
  • Provide and maintain documentation of homeless status for intake workers and clients; keep copies in client files.
  • Provide HIV/AIDS information, education, and linkages to testing, housing, and services; be knowledgeable about HIV/AIDS services and testing locations.
  • Enter activity into HMIS system regularly and assist with collecting and inputting information for LAHSA-required reports (e.g., QPR, APR).
  • Build rapport with community organizations serving homeless individuals, families, low-income households, and other HOPICS target populations.
  • Inform, educate, and refer homeless individuals and families about HOPICS programs and available services.
  • Work evenings, overtime, weekends, or holidays as required.
  • Maintain appropriate boundaries and adhere to SSG’s Code of Ethics and HOPICS’ Core Values.
  • Represent the Agency and Division professionally at meetings and community events.
  • Maintain regular attendance and client files/records in compliance with HIPAA, CFR42, and funding requirements for auditing purposes.
  • Ensure all client records/documentation are submitted or arranged for submission upon resignation.
  • Perform other duties as assigned by the immediate supervisor or Division Director, including support for administrative and data-related IT functions.

Requirements

  • Some credits from an accredited college or university, or at least two years of experience working with homeless individuals.
  • Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.
  • Experience working with homeless clients in the field.
  • Effective written and oral communication skills with collaborative entities.
  • Neat and professional appearance; attention to detail and punctuality.
  • Working knowledge of Microsoft Word, Excel, and the Internet.
  • Experience working with homeless persons with co-occurring issues (mental health, disabilities, substance use).
  • Ability to work with populations presenting with mental illness, health conditions, and substance-related concerns.
  • Ability to report (verbal/written) legal issues such as Tarasoff, child abuse, vulnerable adult abuse, danger to self/others, and grave disability.
  • Professional experience working within a multidisciplinary team.
  • Ability to coordinate submission of client documentation if unable to work.
  • If in recovery, a minimum of three years of being drug and alcohol-free is required.
  • Verification of employment eligibility, background clearance, and TB test (within 3 months prior to or 7 days after hire, renewed annually).
  • CPR and First Aid Certification required within 30 days of employment.
  • Valid Driver’s License and auto insurance required.
  • Knowledge of evidence-based practices and ability to use them in the field.

Working Conditions

  • Work in diverse environments, including homeless encampments, across Service Planning Area 6.
  • Extensive interaction with homeless individuals, those with severe physical/mental disabilities, and individuals with infectious diseases.
  • Potential exposure to aggressive/hostile clients, stray animals, moving vehicles, and high-crime areas.
  • Readiness to deal with traumas that may trigger personal trauma.
  • Collaboration with courts, Department of Children and Family Services, LAHSA, law enforcement, and other community agencies.
  • Local automobile travel required; exposure to weather conditions, noise, moving vehicles, high-crime areas, odors, fumes, and pollen.
  • Work in noisy environments where adults may talk loudly and expressively.
  • Ability to work effectively and produce quality work under pressure.

Physical Requirements

  • Sitting, standing, typing, thinking, writing, walking, and driving.
  • Carrying items up to 25 lbs.
  • Listening and speaking.

Mental Requirements

  • Ability to perform effectively under unpredictable situations, crises, constant distractions, interruptions, and uncontrollable changes in priorities/work schedules.
  • Process information, think, and conceptualize effectively.
  • Manage personal trauma that may be triggered by work situations.
