Submit your application for the opportunity below: Mental Health Professional II Masters Job Summary And Qualifications The Mental Health Professional II is to deliver exceptional, patient-focused care by providing supervised mental health services to patients and their loved ones. Their responsibilities include conducting assessments, offering psycho-education, leading process groups, developing treatment plans, and facilitating therapeutic counseling sessions using evidence-based interventions in one-on-one, family, and group therapy. The Mental Health Professional II facilitates high quality, patient-centered care by providing psychiatric treatment services to patients and their families. The Mental Health Professional II performs assessments, psycho-education, process groups, treatment planning, discharge planning and therapeutic counseling support through evidenced based interventions in individual, family, and group therapy. What you will do in this role: You will assess, plan, intervene and evaluate the care of psychiatric patients. You will instruct patients and families regarding treatment instructionsDemonstrates advanced clinical skills and ability to plan and facilitate individual, group and family psychoeducational and process groups. Develops treatment plans and attends treatment planning meetings and reviews on an ongoing basis. Collaborates with the multidisciplinary team to assess and coordinate patient’s discharge planning needs and readiness for discharge. Provides guidance on complex issues. Conducts post-discharge follow-up as indicated. Facilitates and participates in Community Education, Activities and Support Groups, as appropriate to scope. Requirements: Master’s degree in a Human Services related field (Psychology, Social Work, Mental Health, or CounselingAdvanced educational preparation or experience in the area of clinical practice and/or managementAmerican Heart Association BLS (within 30 days of hire)Crisis Intervention Prevention (CPI) training required (within 30 days of hire) What you will do in this Role: Conducts one-on-one, group, and family psychoeducational and process groups. Develops treatment plans and attends treatment planning meetings and reviews on an ongoing basis. Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition. Identifies risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement). Completes assessments, group notes, and all other documentation promptly and in compliance with requirements, as appropriate to scope. Collaborates with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness for discharge. Conducts post-discharge follow-up as indicated.
