Job Summary And Qualifications Demonstrates advanced clinical skills and ability to assess patients to determine appropriate level of care. Identifies cultural considerations, gender identity and preferred language for patients and familyDemonstrates advance practice in assessments, and all other documentation timely and in compliance with requirements (as appropriate to scope)Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's conditionIdentifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement)Recognizes and acts upon unexpected assessment findingsLeads in ensuring a safe structured milieu with evidence based treatment modalities/resourcesMaintains active and timely communication with patients, families, multidisciplinary team and continuum of care providersProvides education and guidance regarding state commitment processes, abuse reporting requirements, duty to warn and guardianship processes. Assesses, collaborates, coordinates and provides guidance on complex discharge planning needs and readiness for discharge with the multidisciplinary team. Develops and maintains a community resource guide including access to public assistance/alternatives. Facilitates and participates in Community Education, Activities and Support Groups (as appropriate to scope)Conducts post-discharge follow-up if indicatedAssists with Process and Quality Improvement initiativesDelivers patient care based on accepted standards of careDemonstrates appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety.
