About the role
Mental Health Intake Nurse responsible for assessing behavioral health patients at entry points.
Responsibilities
- Conduct psychosocial and risk assessments to evaluate patient needs.
- Gather comprehensive medical and psychiatric histories.
- Triaging patient acuity to determine appropriate care pathways.
- Coordinate safe admissions, transfers, or referrals to appropriate facilities or providers.
- Collaborate with psychiatrists, emergency department and inpatient teams, and social workers to develop initial care plans.
- Ensure accuracy and compliance in documentation and regulatory requirements.
- Educate patients and families about care processes and treatment options.
- Support a safe, therapeutic environment using evidence-based and trauma-informed practices.
