Mental Health Clinician (Licensed)

About the Role

To provide treatment, support, advocacy, and linkage to individuals enrolled in the Personalized Recovery Oriented Services (PROS) Programs, Home and Community Based Services (HCBS), and Community Oriented Recovery and Empowerment (CORE) to empower individuals in their personalized recovery goals. The Behavioral Health Clinician will also provide education and advocacy to individuals and family members as directed by individualized treatment. This position is a supportive member of the interdisciplinary team.

Responsibilities

• Performs initial intake and assessment/evaluation through individual interviews, interviews of significant others, and review of previous records to gather pertinent information.
• Develops a Treatment Plan addressing specific identified needs, including psycho/social skills, self-esteem, depression/suicidal ideation, symptom control, anxiety, and independent living skills.
• Provides social work and other specialty interventions through individual, joint, or group counseling.
• Documents each individual session and writes progress notes, including progress toward long- and short-term goals, changes in Treatment Plan, and other significant events.
• Performs case management services, including treatment and community referrals, advocacy, psycho-education, and crisis intervention.
• Performs periodic evaluations to establish patient needs and determine how identified needs can be met.
• Acts as liaison with other agencies and departments involved in the individual’s care.
• Completes discharge paperwork within regulatory requirements.
• Serves as a coordinator of services for a caseload, including participation in Team Meetings, Assessments, and Treatment Plan formulation.
• Provides specific assessments within scope and expertise to individuals enrolled in behavioral health services.
• Meets all quality and productivity standards as defined by management and regulatory/funding entities.
• Assists with Peer Review Board representation, community living exploration, and maximizing naturally occurring supports.
• Serves as an advocate for family members, legal/forensic support, and outreach and engagement efforts.

Requirements

• Comprehensive knowledge of mental health and the Psychiatric Rehabilitation Model.
• Ability to communicate effectively, both verbally and in writing.
• Self-directed, self-motivated, and well-organized with strong time-management skills.
• Strong understanding of the person-centered approach and knowledge of or ability to learn the philosophy of empowerment.
• Excellent written and verbal communication skills, including the ability to effectively communicate with individuals, families, clinicians, and community supports.
• Willingness and interest in providing direct service to those diagnosed with Severe and Persistently Mental Illness.
• Valid driver’s license and acceptable driving record according to guidelines established by the organization’s insurance carrier or equivalent accommodations.
• Physical condition commensurate with the demands of the job.

Qualifications

Professional clinical staff must be qualified by credential, training, and experience to provide supervision and direct service related to care.
