What You Will Do

  • Counsels patients and their significant others to obtain history and identify preliminary issues for treatment focus.
  • Contacts previous and current tertiary caregivers for relevant clinical data.
  • Develops, implements, and monitors the patient’s individualized treatment plan.
  • Conducts daily group and individual assessments to identify issues for treatment focus.
  • Provides individual, group, and family process-oriented counseling sessions.
  • Conducts educational, aftercare support, and other didactic groups for patients and family members.
  • Participates in regular clinical team meetings and provides input on the psychosocial, disease management, and case management needs of the patient.
  • Demonstrates adherence to all procedures and legislation applicable to patients in a mental health and substance abuse treatment facility.
  • Maintains accurate and timely medical records, including psycho-social assessments, family session, and daily progress notes.
  • Serves as the facility’s representative in legal proceedings related to patient reports.
  • Schedules, attends, and documents court hearings as required.
  • May provide backup services for incoming assessment and referral calls.
  • Participates in activities that enhance professional growth and development.
  • Demonstrates conduct in line with LOH ethical standards.
  • Complies with facility safety, infection control, and security programs.
  • Floats to alternate units or Linden Oaks locations based on census and staffing needs.

What You Will Need

  • Required Education, Skills and/or Experience:
    • Master’s Degree in Social work or a Master’s Degree in a recognized mental health field or a Bachelor’s Degree and completion of an approved substance abuse education program for assignment in the substance abuse area.
    • Knowledge of developmental psychology, disease pathology, and clinical needs of assigned population.
    • Ability to maintain information which is highly confidential.
    • Ability to clearly summarize pertinent clinical information via written correspondence and medical records documentation.
    • Ability to demonstrate self-control in volatile or hostile situations.
    • Ability to set therapeutic limits and maintain therapeutic distance with clients; maintains therapeutic boundaries with patients during treatment and following discharge.
  • Preferred Education, Skills And/or Experience:
    • Minimum of two years of experience in a behavioral health setting.
    • Minimum of three letters of current State of Illinois licensure with a minimum of three letters which may include LPC (Licensed Professional Counselor), LCPC (Licensed Clinical Professional Counselor), LSW (Licensed Social Worker), Associate Licensed Marriage and Family Therapist (ALMFT), LMFT (Licensed Marriage and Family Therapist), LCSW (Licensed Clinical Social Worker), or LCP (Licensed Clinical Psychologist).
    • Current CPR Certification issued by American Heart Association.
    • CPI (Crisis Prevention Intervention) certification within 90 days of hire.
