Mental Health Case Manager - Adult Services | Texarkana, Atlanta and Clarksville Areas

Responsibilities

• Affirms and describes the needs for services and supports.
• Negotiates and facilitates the array of services and supports needed to address the individual's identified goals and desired outcomes.
• Provides behavioral intervention services, training, and support to assigned caseload.
• Safeguards planned services and supports are implemented and appropriately documented.
• Connects individuals with appropriate services and supports, as needed.
• Documents all meetings and notes pertaining to consumer plans.

Qualifications

• Education: Bachelor's or Master's degree in a Human Services field such as psychology, sociology, social work, rehabilitation, counseling, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
• Master's degree preferred.
• Registered nurses also qualify for these positions.
• Certifications, licenses, or registrations may be required.
