About the Role
As a Clare Bridge Program Coordinator, you will plan, coordinate, and facilitate a calendar of person-centered, dementia-friendly programs tailored to the needs of residents in our dementia care community. This role enhances residents' quality of life by engaging them in meaningful activities while honoring their individual abilities, habits, and preferences. Responsibilities
- Design a 7-day-a-week person-centered program that is engaging, creative, and meets the needs and interests of individual residents.
- Promote and engage residents in the 7 domains of well-being programs in a dementia-friendly structured environment, including sensory-focused small group programming for residents who cannot participate in larger groups.
- Develop a monthly calendar in partnership with residents based on their shared interests, ensuring person-centered special events and daily programs are scheduled and executed as planned.
- Ensure dementia care programs comply with Brookdale Excellence Standards Tool (BEST), divisional dementia care site visit tools, state, federal, and other regulations, and align with Brookdale’s mission, policies, procedures, and dementia care standards.
- Collaborate with community leadership to educate and provide resources to families and associates about Alzheimer’s disease and other dementias, including facilitating dementia-focused in-services, family events, and computer-based training such as Understanding Alzheimer’s & Dementia Care (UADC).
- Provide direct supervision of Clare Bridge Program Assistant(s), if required, including hiring, coaching, counseling, and documenting corrective actions. Complete job performance evaluations and act in a supervisory capacity in the absence of the Manager or Executive Director.
- Plan, coordinate, decorate, and participate in special events, including holidays, signature programs, National Walk to End Alzheimer’s, family socials, educational, and marketing events.
- Report changes in residents’ overall health, including behavioral changes, to the clinical partner and collaborate on interventions to address concerns.
- Oversee social media content to ensure alignment with Brookdale’s culture, brand, and social media guidance.
- Plan and schedule programming events outside the community, which may include driving a community motor vehicle.
- Maintain and adhere to the department’s monthly budget and expense control; manage the volunteer program.
- Perform personal care, housekeeping, and dietary responsibilities as needed to maintain service quality in accordance with Clare Bridge Program standards. Complete required training and qualifications based on state regulations.
Requirements
- Bachelor’s Degree in therapeutic recreation, gerontology, education, or a related field preferred.
- Minimum of two years of direct programming experience with older adults and persons with dementia preferred, or an equivalent combination of education and experience.
- Valid driver’s license and a current 3-year driving abstract with no more than one moving violation; may be required to obtain a commercial driver’s license based on community needs.
Skills
- Working knowledge of dementia care and programming for older adults.
- Basic typing skills and proficiency in PC and word processing software, preferably in the Microsoft Windows environment.
- Ability to use limited independent judgment to make decisions based on precedents and established guidelines.
- Strong collaboration and communication skills to work with residents, families, associates, and community leadership.

Pay
Base pay is determined by the applicant’s skills and experience.

- Potential injury from transferring, repositioning, or lifting residents; exposure to falls, burns, odors, or cuts from equipment.
- Occasional travel may be required.
