Care Coordinator

About the role

Traditional Home Care has an immediate opening for a Care Coordinator. The position is salaried and paid weekly, with an aggressive Bonus Plan and a very generous benefit package including Health, Dental, Vision, 401K, Long/Short Term Disability, Paid Time Off from start of service, and a Non-Corporate family-owned & operated atmosphere.

Responsibilities

• Provide superior customer service and quality home care
• Build lasting relationships with clients, referral sources, payers, and community organizations
• Career Development
• Manage and grow your client caseload
• Develop strong, communicative relationships with the team
• Interview, hire, and orient reliable and compassionate caregivers
• Function as primary support for daily EVV and payroll functions

Requirements/Qualifications

• Previous home health care or home care experience highly desirable
• High School or GED required. Associate’s Degree preferred
• Staffing experience preferred
• Excellent verbal/written communication, time management and organizational skills
• Able to effectively multi-task
• Effective problem solving skills
• Strong presentation and training skills
• Strong customer service skills
