Responsibilities Deliver high-quality, compassionate nursing care to residents in accordance with individualized care plansProvide person-centered care to residents living with dementia and other memory-related conditionsSupport residents with activities of daily living (ADLs) while promoting dignity, independence, and respectPromote resident engagement, socialization, and meaningful activities to support mental and emotional well-beingCreate and maintain a safe, secure, and supportive environment for residentsFollow the nursing process to assess, plan, implement, and evaluate individualized careMonitor residents for changes in condition, behavior, mood, or overall well-being and respond appropriatelyAccurately document resident history, vital signs, clinical observations, and changes in conditionAdminister medications and treatments as ordered and in accordance with applicable policies and regulationsCommunicate effectively with RNs, physicians, CNAs, CMTs, families, and interdisciplinary team membersProvide guidance and support to the nursing team to ensure consistent, high-quality resident care Qualifications Current LPN license in the state of Missouri (Required)Previous experience in memory care, dementia care, long-term care, assisted living, rehab, or a similar healthcare setting preferredStrong communication, clinical, organizational, and leadership skillsCompassionate and patient-centered approach to resident careAbility to work effectively as part of a teamCommitment to providing safe, respectful, and individualized care Join Our Memory Care Team!
