About the role

We are seeking a compassionate Licensed Practical Nurse (LPN) to join our Memory Care team. In this role, you will provide personalized clinical care for residents with cognitive impairments, including medication and treatment administration, resident monitoring, and health assessments. You will offer clinical guidance and mentorship to care staff, ensuring high-quality, compassionate support. The position involves collaborating with interdisciplinary teams and maintaining clear communication with families regarding care plans.

Responsibilities

• Administer medications and treatments to residents with cognitive impairments

Requirements

• Active LPN license in Indiana
• Current CPR/BLS certification
• Prior experience in Memory Care or Long-Term Care
