Memory Care Coordinator

Key Responsibilities

  • Oversight of daily operations of the Memory Care Neighborhood.
  • Development, coordination, and leading of engaging memory care programming and activities.
  • Providing visible leadership and support to residents, families, and team members.
  • Supervision, coaching, and training of memory care staff.
  • Collaboration with various departments to ensure seamless resident care.
  • Participation in resident assessments, admissions, move-ins, and care planning.
  • Maintenance of accurate documentation and compliance with regulations.
  • Regular communication with residents' families about care, services, and changes in condition.
  • Aid in budgeting, inventory management, and departmental planning.
  • Participation in the community's on-call rotation.

Qualifications

  • Preferred degree or certification in Medical Assistant or related fields such as Nursing, Social Services, Behavioral Health, or Therapeutic Recreation.
  • Minimum of 4 years of experience working with individuals living with dementia.
  • Minimum of 2 years of supervisory experience.
  • Preferred certifications: Certified Dementia Practitioner (CDP), Certified Alzheimer Disease and Dementia Care Trainer (CADDCT), CPR, and First Aid.
  • Strong leadership, communication, organizational, and problem-solving skills.
  • Knowledge of dementia care best practices and applicable healthcare regulations.
  • Willingness to obtain Medication Technician Certification within 6 months of hire.
