Responsibilities
- Coordination of medication safety activities in the organization
- Leadership for medication safety assessments
- Educate providers and staff on system-based causes for medication errors
- Communicate best practices regarding medication safety strategies
- Interface with hospital site and system pharmacy and quality representatives on medication safety issues
- Review, analyze, and trend all medication events for the system
- Participate in root cause analysis involving a medication event
- Create or review/revise patient care policies impacting the safe use of medications
Qualifications
- Graduate of an accredited school of pharmacy
- Advanced degree (MS or PharmD) and/or residency preferred
- A minimum of five years clinical practice experience in a hospital or health system pharmacy practice (or four years plus a year of residency training)
- Keyboard skills and ability to navigate electronic systems applicable to job functions
- Current Registered Pharmacist License (RPh) for applicable state required
- ACLS and/or PALS required within three months of hire
- Completion of ASHP or ISMP Medication Safety Certificate program within 3 years of hire
Pay
Pay Range: USD $70.21 - USD $104.61 /Hr.
