Position Summary
Works in the Emergency Department to collect admission medication histories. The MRT, with oversight by the Transitions of Care Pharmacist, assists prescribers with the home medication portion of the electronic admission medication reconciliation process. The employee may function as an operational technician when necessary and/or if time allows.
Primary Position Responsibilities
- Collects an accurate and complete home medication history by interviewing the patient and utilizing secondary sources as needed.
- Accurately and completely enters medication history into electronic medical record (EMR).
- Provides operational assistance in the Emergency Department or main pharmacy when necessary and if time allows.
- Assists in training pharmacy technicians pertaining to medication histories.
Qualifications
- Minimum Education: Required: High school diploma or equivalent. Preferred: ASHP/ACPE Accredited Pharmacy Technician Education Program; Associates Degree or higher.
- Required Years of Experience: 0 to 1 year hospital or retail pharmacy experience.
- Preferred Years of Experience: 2 or more years of retail pharmacy experience.
- License, Registry or Certification: Current New Jersey Board of Pharmacy Technician License. Preferred: Certified Pharmacy Technician (Pharmacy Technician Certification Board).
- Knowledge, Skills and/or Abilities: Mathematical proficiency. Dexterity. Successful completion of written and hands-on competencies. Technical expertise (site level) for Automated Dispensing Cabinets (ADC) (e.g. PYXIS® or Omnicell®). Competence in required computer programs (clinical system, ADC, packaging, parenteral documentation). Preferred: MS Office proficient.
