MEDICATION RECONCILIATION TECHNICIAN
About the role
This position is responsible for collecting the current home medication list of patients admitted to Blessing Hospital. This list includes prescription and non-prescription medications administered outside the hospital.
Responsibilities
- Interview the patient and/or significant other
- Gather medication information from the patient’s home
- Document the list of current medications
- Work with the pharmacist to complete an initial review
Requirements
- High School Diploma or equivalent
- Completion of Pharmacy Technician Training Course
- Computer skills required
- Excellent written and verbal communication skills
- PREFERRED: Pharmacology education or pharmacy experience, excellent typing skills
Qualifications
- Illinois registration as a Pharmacy Technician or equivalent certification by the National Pharmacy Technician Certification Board, unless exempted by the state of Illinois due to Grandfathering
