About the Role

This position encompasses two primary roles within Bronson Healthcare:

• Hospital Medical Social Worker (PRN - Bronson At Home): Delivers social work services across all medical areas of the hospital to diverse patient populations. Provides direct intervention in areas such as mental health, suicide prevention, domestic violence, sexual assault, child/adult abuse/neglect, death/grief, crisis intervention, advocacy, and substance abuse. Works with patients and families on discharge planning, educates on community resources, and collaborates with the patient care team to ensure timely and appropriate discharges to psychiatric facilities, substance abuse treatment, and other care settings.
• Practice Medical Social Worker: Delivers care management and behavioral health social work services in ambulatory medical practice settings for pediatric, adolescent, adult, and geriatric populations. Coordinates care to ensure access to community services, administers behavioral health screenings, conducts assessments, and delivers brief interventions. Responds to crises, identifies high-risk psychosocial needs, and collaborates with interdisciplinary teams to address the impact of illness on mental and emotional health. Provides culturally competent clinical services and prevention, treatment, and recovery support for patients with mild to moderate risk factors.

Responsibilities

• Assess the nature of crises and immediate social/emotional needs of patients and families to provide appropriate interventions.
• Perform screenings based on patient population and needs.
• Identify problems surrounding crisis situations and act as a patient advocate.
• Collaborate with interdisciplinary healthcare teams to recommend or provide immediate treatment and solution-focused interventions.
• Develop care plans and make referrals to community agencies and healthcare specialists to ensure follow-up treatment and support.
• Thoroughly document cases and enter information into patient medical records.
• Participate in the development of departmental policies, procedures, and protocols.
• Identify and address learning needs of patients, families, and healthcare coworkers.
• Assume responsibility for professional development and support a positive work climate.
• Mentor and supervise students.
• For Bronson Lakeview Hospital and Bronson South Haven Hospital:
  • Complete the annual "Swing beds" CBL (Computer-Based Learning) to understand documentation differences and specific care requirements for swing bed patients.
  • Participate in recipient rights education and ensure protection of recipient rights as required by the Michigan Mental Health Code.
  • Ensure recipients, parents of minor recipients, and guardians have access to and are notified of their rights.
  • Post conspicuously the contact information for the Office of Recipient Rights and Rights Advisor at all service sites.
  • Maintain a record system for reports of rights violations and ensure secure storage of investigative documents.
  • Visit service sites at least annually to ensure rights protection.
  • Serve as a consultant on recipient rights matters to leadership and staff.
  • Investigate reports of rights violations in accordance with the Michigan Mental Health Code.
  • Provide semiannual summary complaint data and remedial action reports to the State of Michigan and the Recipient Rights Advisory Committee.

Requirements

• Hospital Medical Social Worker:
  • Master’s Degree in Social Work and Licensed Master’s Social Work (LMSW) or Limited License Master’s Social Work (LLMSW) in good standing, OR
  • Master’s Degree in Psychology or Counseling Psychology and Licensed Professional Counselor (LPC), Limited License Professional Counselor (LLPC), Licensed Psychologist (LP), or Limited License Psychologist (LLP).
  • Must be fully licensed within two years of hire, including completion of state-required supervision hours and licensing exam.
  • 1-2 years of professional experience in healthcare or mental health preferred.
  • Current driver’s license in Michigan and proof of insurance required for Bronson At Home (BAH).
• Practice Medical Social Worker:
  • Master’s Degree in Social Work with one year of recent medical or mental health experience, OR
  • Limited License Social Work with at least one year of medical or mental health experience. Must be fully licensed within two years of hire, including completion of state-required supervision hours and licensing exam.
  • Master’s Degree with Limited License Professional Counselor (LLPC) or Licensed Professional Counselor (LPC) credentials.
• Ability to move about the hospital to meet with patients and families in patient care units.
• Basic word processing skills.
• Excellent human relation skills, including verbal and written communication, problem-solving, advocacy, assertiveness, and empathy.
• Customer service orientation with the ability to communicate clearly and effectively with patients, families, and interdisciplinary teams.
• Effective interviewing, problem-solving, and critical thinking skills.
• Regular use of negotiation and conflict resolution skills.
• Work involves high levels of mental/visual fatigue, including sustained periods of concentration on interactive, repetitive, or detailed tasks, and operating a personal computer or CRT 40-70% of the time.
• Regularly lifts bulky or moderately heavy weights (up to 50 pounds) and occasionally assists with heavier tasks or equivalent effort in pushing, pulling, or handling materials and equipment.
