Medical Social Worker - PRN

About the Role

This position is responsible for bio-psychosocial assessments, patient education, evaluation, referral to appropriate community resources, and supportive and crisis interventions. The role applies thorough knowledge and use of company and community-based services to coordinate and implement complex discharge plans. You will collaborate and communicate (verbally and through medical records) with the multidisciplinary team to evaluate and respond to patient and caregiver needs, including emotional, social, cultural, and spiritual needs related to the patient’s medical condition. Additionally, this role may practice clinical social work within the agency under the supervision of a Licensed Clinical Social Worker (LCSW) employed by the company.

Responsibilities

• Conducts patient assessment interviews, provides referrals, and coordinates discharge plans.
• Identifies and provides helpful resources to patients and families.
• Maintains patient records, documents critical information, and communicates with the medical team.
• Maintains professional and technical knowledge and required certifications through continuing education and development.
• Utilizes management resources and participates in on-call rotation.
• Adapts behavior to the specific patient population, respecting privacy, communication style, and cultural needs.
• Remains knowledgeable on current federal, state, and local laws, accreditation standards, and regulatory requirements.
• Other related duties as required.

Requirements

• Master’s degree in Social Work (MSW) from a university accredited by the Council on Social Work Education (CSWE).
• Certified Social Worker (CSW) as verified by the LABSWE or Registered Social Worker (RSW) certificate.
• Preferred: Licensed Master Social Worker (LMSW) as verified by the LABSWE or must schedule and pass the LMSW exam within 6 months of employment.
• Preferred: Two years of experience in a medical setting.

Skills

• Computer skills and proficiency with Windows-style applications, keyboard, and role-specific software packages.
• Effective verbal and written communication skills, with the ability to present information clearly to varying levels of individuals.
• Working knowledge of common disease entities, medical terminology, and the psychosocial and economic effects of illness/trauma on patients and families.
• Knowledge of healthcare systems and public programs (e.g., Medicare, Medicaid, Medicare Advantage, commercial insurance plans).
• Strong interpersonal skills to interact effectively with patients, families, multidisciplinary teams, peers, and community agencies.

Physical and Environmental Demands

• Medium Work: Exerting 20 to 50 pounds of force occasionally, 10 to 25 pounds frequently, and negligible to 10 pounds constantly to move objects.
• Must be able to sit and stand for prolonged periods.
• Must be able to travel throughout and between facilities.
• Flexible work schedule, including 24/7 availability, weekends, holidays, and on-call rotation.
• Potential occupational exposure to blood, body fluids, tissue, hazardous medications, or hazardous waste.
• Risk of exposure to communicable diseases within a healthcare setting.
