Medical Social Worker-PRN

Job Summary and Responsibilities

At Deaconess Home Health, we strive to deliver hope and healing to those we serve. As a Medical Social Worker, your responsibilities will include:

• Establishing a care plan in accordance with the physician's plan of treatment.
• Providing crisis intervention as required.
• Assisting the physician and other caregivers in understanding the significant social, emotional, and economic factors influencing the patient's ability to improve medically.
• Providing patient care education to patients and caregivers.
• Aiding in making appropriate community referrals to gain access to additional resources as needed (e.g., applications for public assistance, housing problems, additional healthcare resources, transportation).
• Working with the patient's caregivers to resolve problems interfering with an optimal caring relationship between the patient and caregiver.
• Completing and submitting all required documentation in an accurate and timely manner.

Job Requirements

• Master’s degree from an accredited school approved by the Council on Social Work Education.
• In Indiana, must also be certified under IC25-23.6-4-3.
• Currently licensed to practice in the state serviced.
• One year social work experience in a health care setting.
• Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements.

Pay Range

$26.70 - $39.71 /hour
