PRN Medical Social Worker

Responsibilities

  • Completes the Comprehensive Assessment within five (5) days of the patient’s election of hospice care to identify psychosocial, financial, environmental, and community needs, as evidenced by Plan of Care (POC) documentation, clinical records, IDG/IDT meeting notes, and community resource referrals.
  • Affords assistance with advance directives, funeral planning, discharge planning, family counseling, and changes in level of care based on patient and family needs.
  • Provides counseling and support to patients and families/caregivers regarding end-of-life issues, death, and grief.
  • Participates in IDG/IDT meetings, contributes to the patient care planning process, and collaborates with the interdisciplinary team to ensure coordinated patient care.
  • Promotes the hospice philosophy and adheres to administrative policies to ensure quality of care.
  • Demonstrates effective interpersonal and written communication skills.
  • Maintains compliance with mandatory continuing education requirements of the hospice organization and licensing board.
  • Maintains working knowledge of public and private eligibility standards and requirements to ensure appropriate services.
  • Accepts clinical assignments consistent with education and professional competence to meet patient needs.

Qualifications & Requirements

  • Master of Social Work (MSW) degree from a school accredited by the Council on Social Work Education (CSWE); OR Bachelor of Social Work (BSW) degree from a CSWE-accredited institution; OR Bachelor’s degree in psychology, sociology, or a related field with supervision by an MSW and at least one (1) year of social work experience in a healthcare setting.
  • Current and valid LMSW or LBSW license issued by the Texas State Board of Social Worker Examiners.
  • Current driver’s license.
  • One (1) year of experience in palliative care or hospice preferred.
  • Working knowledge of community resources.
  • Strong interpersonal skills.
  • Proficient with computers and basic technology.
  • Reliable transportation and valid auto liability insurance.
  • Ability to remain calm under stressful conditions.
  • Ability to provide counseling and education to terminally ill patients and their families.
