Medical Social Worker - PRN

About the role

We are seeking a Medical Social Worker to provide coordinated home care services across all age groups.

Responsibilities

• Conduct psychosocial assessments, perform initial client evaluations, and develop plans of care that address psychosocial, financial, and environmental needs.
• Collaborate with interdisciplinary teams to ensure seamless patient care, participate in case conferences, and maintain effective communication with clients, families, and healthcare professionals.
• Assist with client admissions, referrals, and coordinate community resources.
• Demonstrate ongoing professional growth through continuing education and competency development.

Qualifications

• Bachelor's or Master’s degree in Social Work
• Current MSW certification
• Valid driver’s license
• Reliable transportation
• Auto liability insurance
• CPR certification
• Minimum of one year experience in healthcare settings

• Excellent interpersonal, communication, and decision-making skills
• Ability to handle stressful situations calmly and effectively

Work Environment & Physical Requirements

Primarily office-based with local travel required, handling prolonged sitting, standing, and walking. Must be available for emergencies and work under some stressful conditions.
