About the Role
The Medical Social Worker uses social work expertise and clinical knowledge to educate patients living with kidney disease on treatment, health, and wellness options to improve quality of life.
Responsibilities
- Conduct psychosocial assessments
- Participate in care planning
- Manage services to support patients' social and emotional well-being
- Help patients cope with chronic kidney disease and dialysis treatments
- Play a vital role in delivering compassionate, individualized patient care
- Collaborate with a multidisciplinary team to enhance patient outcomes
- Set a positive tone that promotes holistic patient well-being
- Seek opportunities for professional growth and team leadership
Requirements
- Master’s Degree in Social Work from an accredited institution
- MSW required; LCSW/ACSW preferred (LCSW in CA)
- One year of experience in behavioral or healthcare settings preferred
- Licensure requirements vary by state (LMSW, LCSW)
