Location: Las Vegas Northeast and Northwest Responsibilities Assess social and emotional needs related to the patient’s healthSupport physicians and clinical team members with psychosocial insightsParticipate in care planning (Plan of Care / POC)Observe, document, and report community resource needsEducate patients and families on available community servicesProvide counseling and guidance to patients and caregiversUtilize and coordinate appropriate community resourcesAssist with discharge planningParticipate in in‑service training and educational programs. Qualifications Master’s degree from a Council on Social Work Education–accredited programMinimum 1 year of MSW experienceStrong physical stamina and mental healthPre‑employment health clearance (TB test/CXR as required by state)Able to read, follow instructions, and accurately document careSelf‑directed; able to work independentlyCurrent CPR certificationProfessional, patient, and flexibleExcellent personal hygiene and communication skillsValid driver’s license and insured vehicle per Lorian Health requirementsAble to verbally communicate in the patient’s language and document all care Why Join us? Pay per visit ranges from $65 to $85 visit commensurate with experience and depending on type of visit.
