About the role

The Medical Records Clerk works closely with front office personnel, providers, and staff to initiate health records charts, maintain participant files and statistics, respond to requests for medical records, and perform clerical duties.

Responsibilities

  • Organize and maintain health records by collecting participant information, including test results, diagnoses, exam results, recommended treatments, prior medical history, and other relevant data.
  • Maintain and retrieve participant files and documents for scheduled appointments; initiate records for new participants, create computer indexes, prepare file labels, and maintain filing statistics and audits.
  • Adhere to professional standards, High Desert PACE policies and procedures, federal, state, and local requirements.
  • Accept ownership for new and different requests and explore opportunities to add value to job accomplishments.
  • Serve as backup coverage for front desk staff.
  • Keep supervisor informed of problems or issues.
  • Retrieve participant schedules from the computer system; distribute lab reports to physicians and materials to other departments; respond to correspondence requests.
  • Participate as a member of the interdisciplinary team in assessments (initial, semi-annual, unscheduled, and annual) and attend morning updates to report changes in participants' baseline status.
  • Involve in the development and implementation of QAPI (Quality Assurance and Performance Improvement) activities.
  • Attend and participate in staff meetings, in-services, projects, and committees as assigned.
  • Adhere to and support the center’s practices, procedures, and policies, including assigned break times and attendance.
  • Accept assigned duties cooperatively and perform all other related duties as assigned.
  • Be flexible with the schedule of hours worked.
  • May require the use of a personal vehicle.

Requirements

  • Minimum of one (1) year of documented experience working with a frail or elderly population.
  • High school diploma or equivalent.
  • Current driver’s license in the state of California.
  • Medically cleared for communicable diseases and up-to-date immunizations before engaging in direct participant contact.

Skills & Abilities

  • Proficient knowledge of computer skills, MS Office (Word, Excel, Access, PowerPoint, and Outlook).
  • Knowledge of general office procedures, equipment, and filing systems.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to quickly learn department policies, procedures, goals, and services.
  • Attention to detail and accuracy.
  • Ability to change priorities regularly.

Working Conditions

  • Ability to access all areas of the center throughout the workday.
  • Ability to lift up to 35 pounds occasionally, 15 pounds frequently, and 7 pounds constantly; required to obtain assistance for objects over 25 pounds.
  • Requires a valid driver’s license.
