Medical Practitioner (Post-acute)

Responsibilities

• Perform comprehensive assessments and manage acute and chronic conditions in complex, medically fragile populations.
• Utilize telehealth and evidence-based strategies to enhance patient outcomes and improve the overall care experience.
• Act as the vital liaison between hospitals, families, and facility leadership.
• Be the "go-to" expert for nursing staff and therapy teams.
• Champion quality metrics and cost-effective medicine that prioritizes the patient’s goals over everything else.

Qualifications

• A Master’s or Doctorate from an accredited program (NP or PA) with a current state license, Board Certification, and an active DEA.
• A background in Geriatrics, Post-Acute, or SNF settings is strongly preferred.
• You understand the unique rhythm of the post-acute environment.
• You are independent yet collaborative, a master of time management, and a compassionate communicator who treats every patient with profound respect.

Pay
