Job Description

Assessment & Management

  • Performs complete history and physical examinations and documents findings in the medical record.
  • Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information, and indicated diagnostic procedures.
  • Serves as the first responder for both nursing and house staff in the event of patient status changes, including placing orders and initiating procedural and resuscitative interventions.
  • Affords assessments of risks associated with the care of acutely ill and complex chronically ill patients, including: medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions, and diagnostic procedures.

Diagnosis

  • Collaborates with the interdisciplinary healthcare team in the diagnosis of acute and complex chronic conditions.
  • Manages diagnostic testing, including ordering, interpretation, performance, and supervision.
  • Formulates differential diagnoses in order of priority.
  • Identifies complications and implements appropriate interventions.

Plan of Care

  • Identifies expected outcomes based on diagnosis and develops, documents, and manages a comprehensive plan of care addressing complex acute and chronic healthcare needs.
  • Utilizes evidence-based practice guidelines and protocols to create individualized, dynamic care plans across the continuum.
  • Implements and adjusts the plan of care as needed.
  • Prescribes diagnostic strategies and therapeutic interventions.
  • Makes advanced procedures consistent with privileges and competency validation.

Communication and Collaboration

  • Maintains effective communication and collaboration with the interdisciplinary healthcare team.
  • Engages and communicates with patients, families, and staff to promote continuity of care across inpatient and outpatient settings.

Documentation

  • Documents and/or dictates key components of patient care, including: history & physicals, daily progress notes, plan of care, problem lists, procedure notes, acute event notes, transfer summaries, discharge summaries, clinic notes (as applicable), and death summaries in the medical record per unit or service requirements.
  • Ensures documentation is timely, meets acute care compliance standards, and accurately reflects patient acuity.

Qualifications

  • Master’s Degree
  • National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant
  • BLS Certification

Preferred Qualifications

  • Prior experience in Bone Marrow Transplant (BMT) or hematology/oncology setting
