About the Role

This position involves coordinating medical services and medication management for individuals in collaboration with Registered Nurses (RN) and Residence Managers, ensuring compliance with OPWDD regulations and agency policies.

Responsibilities

  • Collaborate with the RN to schedule medical appointments in a timely manner and prepare clinical consultation forms.
  • Coordinate with the RN and Residence Manager to ensure sufficient staffing for appointments.
  • Accompany individuals and/or staff to medical appointments when possible.
  • Review, obtain signatures for, and file all medical documentation in the individual’s medical binder.
  • Ensure all medical appointments and lab work are up to date.
  • Document and implement all physician recommendations.
  • Develop and maintain a weekly/monthly medical appointment calendar.
  • Maintain AMAP certification.
  • Submit prescriptions to ChemRx Pharmacy, count/record medications upon delivery, and verify proper counting by AMAP staff.
  • Complete "Client-Specific Sheet" for new medications.
  • Check Medication Cart/Room weekly to prevent expired medications and ensure a minimum five-day supply; reorder as needed.
  • Ensure ChemRx Pharmacy receives all original prescriptions.
  • Review and log all controlled substances, validate counts minimally weekly, and ensure compliance.
  • Collaborate with RN to discontinue medications as indicated by the prescribing physician.
  • Maintain cleanliness and orderliness of the Medication Room/Cart, ensure sufficient supplies, and order as needed.
  • Ensure all AMAP signatures are on the Medication Administration Record (MAR) before the first administration each month; file MAR documentation monthly.
  • Prepare off-site medications (e.g., home visits, trips) and document in accordance with OPWDD regulations.
  • Assist RN in maintaining individuals’ medical binders and completing forms (e.g., infection control, medical appointment, nursing report, resident report).
  • Complete all paperwork as requested by the RN or Residence Manager.
  • Schedule and complete quarterly pharmacy reviews.
  • Review and monitor Bowel Movement charts daily; report issues to the RN and Manager.
  • Perform additional duties as requested by the RN, Residence Manager, Program Coordinator, or Program Director.
  • Assist individuals with morning routines (ADLs, making beds, breakfast clean-up, meal preparation).
  • Assist with dinner preparation, afternoon routines (e.g., lunch prep, tooth-brushing, laundry), and food shopping as time permits.

Requirements

  • Comprehensive knowledge of OPWDD regulations.
  • Strong organizational and interpersonal skills.
  • Good oral and written communication skills.
  • Valid NYS Driver’s License.
  • Computer literacy.
