About the role
This position involves coordinating medical appointments and necessary communications for individuals living in residential sites, ensuring they receive proper medical care. The role also includes providing direct support and medical supervision.
Responsibilities
- Assist people in receiving needed medical services and document pertinent information (e.g. medication administration, transportation).
- Provide medical supervision and review MARs for refusals daily.
- Track injuries and falls, send information to medical providers, and track monthly MAR renewals to the pharmacy.
- Ensure adequate medication availability, communicate with residential site staff, and make recommendations to improve health and wellness.
- Coordinate with CSRN for Pharmacy Reviews, review medical charts, and implement Life Plan and Staff Action Plans.
- Support individuals in accessing community resources and assist with household tasks, including meal preparation and medication management.
- Complete required paperwork and document residential habilitation services according to agency and OPWDD requirements.
- Drive agency vehicles when needed and complete transportation paperwork.
- Maintain open and positive communication with service recipients, their advocates, family members, and other agency staff.
- Work independently and as part of a team, adhere to program policies and procedures, and participate in compliance activities.
- Participate in corporate compliance, quality assurance, and improvement activities as required.
- Adhere to the DSP Code of Ethics and demonstrate positive role modeling.
Nonessential Functions
- Provide positive role modeling.
- Participate on agency committees and attend agency functions.
Knowledge, Skills, and Abilities
- Good listening and communication skills, strong organizational and problem-solving abilities, basic computer skills, and experience with individuals with developmental disabilities or elderly.
- Positive interpersonal skills and leadership abilities, ability to work independently and as part of a team.
Physical & Mental Requirements
- Physically able to assist individuals in transfers and prevent falls.
Minimum Qualifications
- High School Diploma or equivalent.
- At least one year experience with individuals with developmental disabilities preferred.
- Certified Nursing Assistant or Certified Home Health Aide preferred.
- Valid NYS driver's license with a safe driving record per agency policy.
Working Conditions
- Office equipment and household appliances used.
- Offsite travel to appointments, events, and outings.
- Monday through Thursday 7am-3pm, Sunday 8am to 4pm, 37.50 hours.
