Responsibilities
- Manage a caseload of MAPs participants
- Collaborate with individuals to develop a Virtual Community Resource Map (VCRM)
- Train participants and their support networks on enabling technologies
- Coach participants in independent living skills, job readiness, transportation, healthy living, and social engagement
- Ensure all required documentation is completed accurately, thoroughly, and in a timely manner
- Attend conferences, seminars, and required trainings as assigned
- Cooperate fully with licensing authorities and departmental staff during inspections, inquiries, or investigations
Requirements
- High school diploma or equivalent
- Minimum of two (2) years of full-time experience working in the IDD field or a related human services field
- Strong ability to communicate effectively and professionally with individuals at all levels, both internally and externally
- Strong proficiency in the use of technology and digital tools
- Ability to establish and maintain supportive, respectful relationships with individuals served
- Ability to read, write, and communicate effectively in English
