MDS-RN/LPN-Full Time

About the role

Azria Health Longview is seeking an MDS Coordinator for their long-term care/skilled nursing facility. The ideal candidate will have clinical reimbursement, care planning, MDS 3.0, and case mix experience in a nursing environment.

Responsibilities

• Develop and implement care plans based on MDS assessments
• Ensure accurate documentation of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive resident care
• Monitor and report on resident outcomes and quality metrics
• Provide education and training to staff on MDS processes and care coordination

Requirements

• Licensed RN in the state of Iowa or a compact nursing license
• Strong communication and interpersonal skills
• Self-motivated with a professional and enthusiastic attitude
• Experience with clinical reimbursement, care planning, MDS 3.0, and case mix

Skills

• Proficient in MDS 3.0 assessment and documentation
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong organizational and time management skills
