MDS Nurse (LVN/ RN)
About the role
Misty Willow Healthcare and Rehabilitation Center is seeking a MDS Nurse to join their team. The position is available Monday through Friday, full-time, from 8am to 5pm. The ideal candidate must hold a Texas LVN or RN license and have at least one year of experience as a MDS nurse in a long-term care facility.
Responsibilities
- Assessing and documenting patient needs using the MDS assessment tool
- Collaborating with interdisciplinary teams to develop and implement care plans
- Providing direct patient care based on the care plan
- Communicating effectively with patients, families, and other healthcare providers
- Ensuring compliance with regulatory requirements and facility policies
Requirements
- Current Texas LVN or RN license
- At least one year of experience as a MDS nurse in a long-term care facility
- Strong communication and interpersonal skills
- Proficiency in computer systems used for documentation and care planning
Qualifications
- Passion for providing compassionate care
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Willingness to learn and adapt to new technologies and procedures
