MDS Nurse (LVN/ RN)

About the role

Misty Willow Healthcare and Rehabilitation Center is seeking a MDS Nurse to join their team. The position is available Monday through Friday, full-time, from 8am to 5pm. The ideal candidate must hold a Texas LVN or RN license and have at least one year of experience as a MDS nurse in a long-term care facility.

Responsibilities

• Assessing and documenting patient needs using the MDS assessment tool
• Collaborating with interdisciplinary teams to develop and implement care plans
• Providing direct patient care based on the care plan
• Communicating effectively with patients, families, and other healthcare providers
• Ensuring compliance with regulatory requirements and facility policies

Requirements

• Current Texas LVN or RN license
• At least one year of experience as a MDS nurse in a long-term care facility
• Strong communication and interpersonal skills
• Proficiency in computer systems used for documentation and care planning

Qualifications

• Passion for providing compassionate care
• Ability to work independently and as part of a team
• Strong organizational and time management skills
• Willingness to learn and adapt to new technologies and procedures
