MDS Nurse Full-Time

Gateway Post Acute is seeking a knowledgeable and organized MDS Nurse – Licensed Vocational Nurse (LVN) or Registered Nurse (RN) to join our interdisciplinary team. This full-time position is ideal for a detail-oriented nurse committed to accurate resident assessments, regulatory compliance, and high-quality care.

Responsibilities

• Coordinate and complete accurate MDS assessments within required timeframes.
• Collect, review, and verify resident clinical information.
• Ensure MDS documentation complies with federal, state, and facility requirements.
• Monitor assessment schedules and maintain timely completion of all required documentation.
• Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members.
• Participate in care plan meetings and assist with the development of individualized resident care plans.
• Review medical records to ensure documentation supports resident needs, services, and reimbursement.
• Identify documentation concerns and provide education to clinical staff as needed.
• Maintain the confidentiality of all resident and facility information.
• Support quality assurance and performance improvement initiatives.

Qualifications

• Current and active California LVN or RN license.
• Previous experience in skilled nursing, long-term care, or post-acute care preferred.
• MDS and care planning experience preferred.
• Knowledge of Medicare, Medicaid, PDPM, and regulatory requirements is a plus.
• Strong organizational, communication, and time-management skills.
• Ability to work effectively with an interdisciplinary healthcare team.
• Excellent attention to detail and commitment to accurate documentation.

Pay

• LVN: $72,000–$78,000 per year
• RN: $82,000–$100,000 per year

Compensation is based on experience and qualifications.
