About the Role
This position involves ensuring appropriate reimbursement for patient care services and participating in the Patient Assessment Process, including completing the MDS and Care Area Assessments (CAAs).
Responsibilities
- Assist in developing individualized patient care plans.
- Identify patient needs and outcomes.
- Contribute to the interdisciplinary Resident Assessment Instrument (RAI) process.
- Follow RAI guidelines for completion and transmission of MDS.
- Coordinate assessments based on patient condition changes.
- Support Activities of Daily Living training for staff.
- Maintain knowledge of current RAI/Prospective Payment System regulations through ongoing education.
Requirements
- Must be a licensed Registered Nurse in good standing.
- Experience in healthcare settings such as hospitals or long-term care facilities.
- Prior experience with completing MDSs and Care Plans.
- Knowledge of rehabilitative, restorative nursing practices, and relevant laws and regulations.
