MDS Coordinator, RN

About the Role

This position involves ensuring appropriate reimbursement for patient care services and participating in the Patient Assessment Process, including completing the MDS and Care Area Assessments (CAAs).

Responsibilities

• Assist in developing individualized patient care plans.
• Identify patient needs and outcomes.
• Contribute to the interdisciplinary Resident Assessment Instrument (RAI) process.
• Follow RAI guidelines for completion and transmission of MDS.
• Coordinate assessments based on patient condition changes.
• Support Activities of Daily Living training for staff.
• Maintain knowledge of current RAI/Prospective Payment System regulations through ongoing education.

Requirements

• Must be a licensed Registered Nurse in good standing.
• Experience in healthcare settings such as hospitals or long-term care facilities.
• Prior experience with completing MDSs and Care Plans.
• Knowledge of rehabilitative, restorative nursing practices, and relevant laws and regulations.
