About the role

Responsibilities
- Cook up and complete MDS assessments from start to finish, ensuring accuracy and timely submission
- Manage daily and weekly MDS schedules (ARDs), tracking due dates, and preventing delays
- Inspect resident charts, therapy notes, and nursing documentation to ensure comprehensive assessments
- Collaborate with nursing, therapy, dietary, and social services to gather necessary information and align documentation
- Participate in care plan meetings to help tailor plans to residents' needs and goals
- Identify and address missing or inconsistent documentation before submission
- Monitor Case Mix (CMI) and support accurate reimbursement by ensuring documentation aligns with services provided
- Aid in audits, surveys, and quality reviews by maintaining organized, compliant, and survey-ready documentation
- Support staff in learning best practices for improved accuracy and outcomes
- Act as a primary point of contact for all MDS, care planning, and regulatory compliance matters
Requirements
- Current, active Registered Nurse (RN) state license OR LPN actively enrolled in an RN program
- Long-term care experience preferred
- Prior MDS Coordinator experience preferred, but training is available for the right candidate
- Strong written and verbal communication skills
- Detail-oriented, organized, and deadline-driven
- Positive, dependable, and team-focused attitude
- Excellent attendance and reliability
